肝动脉化疗栓塞治疗原发性肝癌存活20年一例

【摘 要】肝动脉化疗栓塞（TACE）治疗原发性肝癌已成为无法手术患者的首选疗法,我们用此法治疗1例原发性肝癌患者,经多次栓塞,患者存活20年,最终死于心肌梗死,现报道如下。

8 ． V01.17 ， No.9圈栓塞可 以 进行而不会发生瘤壁破裂 。 术前评估动 脉瘤周 围血栓形 成非常重要 ， 而 MRI 比 CT 可 以 更 好的评估动脉瘤 的特征 。 作 者建议不但要仔细 的通 过造影 和 3D-CTA评估动脉瘤 的 内部形 态 ， 同时要通 过 CT或 MRI 中血管 流 空 影 或血肿 、 机化血栓 的形 态辅助评估 动脉瘤 的外部形 态 ， 判 断是 否 有 血栓覆盖于动脉瘤上I15J。 4.7通 过颞 浅 动脉 和 大脑 中动脉 的旁路移植 手术 孤立 动脉瘤 、 颈 内动脉颈段 的血管 内闭塞伴或 不 伴 有旁路手术等也被用 于治疗 BBA 。当最 初 急 性期 (≤ 3d) 动脉瘤 是 血 泡 样 时 ，应 该通 过适 当的外科手术来治疗 ， 因 为这 个病变有非 常高 的快速 生 长 和再 出血 的风 险 ， 亚急性期 (3~7 d) 假性 动脉瘤 的治疗 是 可行 的 ， 因 为病变 的瘤壁 由于 成纤维 细胞 和其他元素包 含 在 内而 变得坚 固 。 在慢 性期 (>7d) 当病 变 已 进 展 为有 瘤囊 出现且 有 表 面血栓形成 时可 以 选择 弹簧 圈栓塞治疗 。 总 之 ， 目前 国 内外研 究关 于 颈 内动 脉血 泡样 动脉瘤 的最 佳治疗方案仍 意 见不一，在 动 脉瘤破裂后 的不 同时期选择适 合的方案进行综合治疗是有效 的 。 [ 参 考 文 献 ] [4][5][6][7][8][9][10] [1] SatohA,NakamuraH,OdakiM,etal.Highrisk aneurysmof [11] che intemalcarotid artery: dorsal lC aneurysm[J].SurgCereb Stroke,1993,21:467-472.
